What anonymous actually means here
The word gets used loosely. These are the four places where your name normally shows up when you get tested in the Netherlands, and what happens to each one with a home test.
Your medical file
A GP records the test and the result in your file, which follows you to any practice you register with later. A home test is not connected to that file at all, so there is nothing to follow you.
Your insurer
Lab work ordered by a GP is claimed through your insurance and appears on your statement, and it uses up your deductible. You pay a home test yourself, so no claim exists.
The delivery
The package carries no product name and no clinic logo. If you share a hallway, a mailbox or a flat with four other people, nothing about it stands out.
The payment
The description on your bank statement is neutral. It does not spell out what you ordered, which matters if you share an account with someone.
One honest exception: a positive result for some infections is reported to the health authorities as an anonymous statistic, without your name. That is how the Netherlands tracks outbreaks, and it applies to every route, including the GGD.
